NOTE : You do not have to be a member of Freedom Doctors Direct Primary Care to undergo an CDL/DOT Medical Exam with Dr. Palmer.  For patients with an active membership the CDL/DOT exam is offered at a discounted price.

Pre-Visit Checklist

Before you present for your DOT physical medical exam…

  1. Bring valid photo identification.

  2. Be at your best:  Be well-rested, and avoid alcohol, caffeine, salt, sugar, and large portions of carbohydrates eight hours prior to your exam

  3. Know your Medical History:

    1. Know your past medication conditions, current medical conditions, and surgical history

    2. Know the name, address, and phone number of current physicians

    3. Medications: Know the names and dosage of all prescriptions medications, over-the-counter medications, and supplements.

  4. Vision: Do not forget your glasses or contacts!  You will need 20/40 vision in both eyes (with or without corrective lenses). If you think you might need updated corrective lens to pass the vision test (make an appointment with your optometrist in advance with enough time for your new prescription to come in.

  5. Hearing: Do not forget your hearing aids!

  6. Blood Pressure: We recommend testing your blood pressure prior to your exam.  Blood pressure readings above 160/100 may prevent you from being certified.

  7. Please bring a copy of the previous DOT physical form and certificate.

FAQs

What is a DOT / CDL physical?

DOT physical exams are intended to determine a driver’s fitness to operate a commercial motor vehicle. The exam including review of medical history, medications, physical exam, vision, hearing, urinalysis. This is not intended to replace regular health evaluations by your primary care provider.

Does my medical insurance cover DOT exams?

No, most insurance plans do not cover DOT physicals. Payment for the exam is your responsibility at the time of service.

Cost of a DOT physical?

CDL/DOT physical exam is $150. Payment options include cash, check or card.

Do I need a urine drug test?

This is not a requirement of the DOT exam, but my be a requirement by your employer. However, you will be required to provide a urine sample to look for protein, blood, sugar to screen for other conditions. Urine drug test available upon request for additional cost ($30).

What happens after the exam?

The doctor will inform you of the results of your examination and any follow-up steps that need to be taken. You will be provided with your certificate for your record
